Dr. Agarwal is Fellow, Foundation of Detection of Early Gastric Carcinoma, Tokyo; WHO Fellow, Mahidol University, Thailand; and has been the National Consultant WHO India.

He harbours deep and cohesive ties with a large number of public institutions both in the governmental and non-governmental sector. He has been associated with NBEMS, UPSC, UGC, AIIMS, NMC, DMC, and NCERT in various capacities. He has been adviser, author and editor to major publishing giants like Elsevier, Wolters Kluwer, Lippincott, Williams and Wilkins, Rupa & Co., Rajkamal, NBT, and Orient; and has been a board member of several philanthropic bodies and national juries.
He has also been Professor and Head, Department of Radiodiagnosis at Vardhman Mahavir Medical College & Safdarjung Hospital, where he guided more than 90 postgrad and post-doctoral students between 1995 and 2024.

He has authored a large body of thought-provoking editorials, seminal papers, essays on history of medicine, and penned pieces of poetry on ethics in medicine. He has been Editor-in-Chief of a national medical journal, and Asia editor of some of the most reputed international journals. He has authored more than 60 books, which have sold more than 3 million copies, penned hundreds of articles, radio and television serials, been a regular contributor to Manorama Year Book, and been a columnist with leading national dailies since 1981 both in English and Hindi.

He is Chair of several Ethical Committees, member of National Committees and a part of the think-tank of a number of national bodies.
